Patient Portal 

Enchantment Healthcare is excited to offer patients usage of a secure Patient Portal to communicate with our office easily and safely through the Internet. At Enchantment Healthcare we believe you should not have to negotiate with the front office or a complicated phone tree to communicate with your doctor. You should not have to wait on hold just to get the first available appointment that may be weeks away. By using the latest technology in the form of a secure patient portal, you can get what you need quickly by securely messaging your provider, including prescription refill requests, lab requests and routine questions. During our business hours these messages are dealt with the same day and quickly. 
Using the Patient Portal, at your convenience day or night, you can:
*Request an acute same day appointment 
* Keep track of appointments
*Access and view lab results
*Request prescription refills
*View your personal health record
*View billing statements
*Update demographic information
*Exchange messages with your provider or the medical assistant 

Instructions: 
Step 1: register with the office at time of check in at your first appointment
Step 2: Open the email sent to you containing the link to the patient portal and your temporary password https://mycw47.eclinicalweb.com/portal6712/jsp/login.jsp
Step 3: Once in the patient portal home screen sign in at the upper R hand corner using your given sign-in and password
*Do NOT click on the blue pre-register button in the middle ifyou have already been given a temporary password as this will create a 2nd account for you 
You can pre register prior to your first appointment by using the pre-register button

What should I do before each visit?
1. Sign on to your Portal account.
2. Under My Account there is an option for Pt Demographics, to enter your address, phone number, and emergency contact information.
3. Under My Account there is an option for Additional information, to enter your pharmacy and additional contacts.
4. Under Questionnaires there are options for Surgical and Allergies and Patient History. Please enter your patient history.

I did not receive an e-mail with my username and password.
Allow up to 24 hours to receive your e-mail. The timing is based on volume and server load. Inform your provider's office if it's been more than 24 hours so someone may help you.
How do I obtain a username and password?
Contact your provider's office to establish a Patient Portal account.
I'm having trouble signing on. What can I do?
1. Confirm that your username is correct. Your username can be found in an e-mail with the subject Portal Login Information.
Or, click Forgot password and enter your username.
2. Be sure you are typing your password correctly. Your password is case-sensitive (the username is not).
3. If you are still having trouble, click the Forgot Password link. You must know your username. You will receive an e-mail with a link to reset the password.
4. If you are still having trouble, please contact your provider's office so they may help you.
How do I change my username?
The username is assigned by your provider's office. Please contact them if you want to change your username.
How do I change my password?
Sign on to the Portal with your existing password, then click the Reset Password option under My Account.
I forgot my password.
Click the Forgot Password? link at the top of the window.
How can I update my address or phone number?
Sign on to the Portal, then click the Pt Demographics option under My Account.
How can I update my emergency contacts or pharmacy?
Sign on to the Portal, then click the Additional Information option under My Account.
How can I send a message or an e-mail to the practice?
Sign on to the Portal, then click either the Message the Practice or Ask the doctor option under My Account.
Contact the office directly by phone if you are having problems or need an appointment.
How can I request an appointment?
Sign on to the Portal, then click the New Appointment option under Appointments.
How can I cancel or request to re-schedule an appointment?
Sign on to the Portal, then click the Recent Appointments option under Appointments. Click Cancel or Re-schedule as needed.
Your practice may not allow online cancellations or rescheduling. If the option does not display, call the practice to cancel or reschedule an appointment.
How can I request a referral or view my referrals?
Sign on to the Portal, then click the View Referrals option under Messages.
How can I see my billing statements?
Sign on to the Portal, then click the Latest Statement or Past Statement option under Account.
How can I view my lab results?
Sign on to the Portal, then click the Latest Statement or Past Statement option under Account. Click the test name, or click View All Results to view them. You can also find your lab results under Medical Record by selecting Lab Diagnostic Reports.
How can I view my medication list?
After you sign on to the Portal, all of your medications will be displayed under Current Medications on the Welcome page. Click the medication name or View all Meds to view them.
How can I refill a medication?
Sign on to the Portal, then click the Refill Medications option under Messages.
How can I update my medications?
For questions or concerns about your medication list, use either Message the Practice or Ask a doctor from the Portal, or contact your provider's office directly by phone.
How can I update my emergency contacts or pharmacy?
Sign on to the Portal, then click the Additional Information option under My Account.
How can I view my immunizations or vital signs?
Sign on to the Portal, then click the PHR View option under Medical Records.
Note: These optional features are configured per practice, and some practices may not offer this feature. If these selections are not displayed, contact the office directly to request the information.
How can I view my allergies, medications, and problem list (Visit Summary)?
Sign on to the Portal, then click the Visit Summary option under Medical Records. Click the date of the visit to see allergies, medications, problems, and lab results from this visit.
