What do I need to do to become a patient?
If you wish to be seen for primary care, please call your insurance and request a change in your PCP to Sabrina Johnson PA-C.
If you wish to be seen for dermatology only and keep your current primary care provider, you will need a referral from your PCP (if required by your health insurance).
If you wish to be seen for aesthetics (cosmetic procedures that are not medically necessary) you will not be able to use your health insurance. You will need to schedule a consultation appointment for $55. The price of your consult will be credited toward any aesthetic procedure that is performed.
If you wish to be seen for cannabis certification, please call the office to help request your medical records prior to scheduling an appointment

If you are well and not ill, but have chronic medical issues you will be required to do Telehealth visits for your appointments.

If you feel you are sick and need medical advice or evaluation, please call and our staff will evaluate the need for an in office visit.
